Survey for Parents of Children Turning 3 or 4 by September 1, 2008

The Creative Learning Center preschool at First United Methodist Church Grapevine is considering expanding services for three and four year olds in the 2008-2009 school year. In addition to the current two-day classes, CLC is evaluating the need for 3, 4 and 5 days per week.

Please indicate your preference for three and four year old classes by choosing the best option for your child/children in the list below:

Three Year Olds

2 half days each week at $140 per month    1 extended day $190    2 extended days $240

3 half days each week at $180 per month    1 extended day $230    2 extended days $280

Four Year Olds

3 half days each week at $180 per month    1 extended day $230    2 extended days $280

4 half days each week at $230 per month    1 extended day $280    2 extended days $330

5 half days each week at $275 per month    1 extended day $325    2 extended days $375
